Project Initialisation Phase in Target Research Projects for Development of Medical Devices

Abstract
The embryo of a new product is always its idea. In the Medical Device development, after generation of the idea, a pre-study phase begins. In many cases, the pre-study phase is dedicated to the development of a new technologyor to the adaptation of an existing technology. These technology development activities are focused on the research into applied product functionalities. In the context of the Medical Device development these activities will be called "target research". The target research significantly extends the duration of the pre-study phase. Many Medical Device pre-studies are complex Cooperation project between different partners. The product idea that has been developed in an enterprise (small or middle-sized) or a university (technical or medical), needs moreover to be revised by one or more project partners in order to further develop or adapt the technology, as well as for the development of devices, or for testing (animals or human trials with histological phases). A simple pre-project has become in this case a complex phase implying target research. Therefore, we propose to add an initialization phase before the pre-study phase. The starting point of the project initialization is the product idea developed by the leading team. The initialization phase is structured in four main sub-processes: idea-refinement, concept and market exploration,partner identification and Cooperation planning. The developed guideline supports a project team during the first stage of any target research project on radical innovation of medical devices. Usually, the idea generator and the initiator of a radical innovation project is the lead user which possibly collaborates with a technical partner (universities). In general, the project must reach its early stage without the Support of a manufacturer. The early stage is focused on the development or the adaptation of a technology for its specific functionalities. The early stage with its significant target research contents has a medium or long duration and has to be elaborately planned in all its different aspects: the choice of Cooperation partners (to make up for lack of knowledge and resources), the management of intellectual proprieties, the first important market researches, the project planning (including clinical trials, design controls, market research,..), the communication planning and the information exchange inside and between the project members. The guideline describes the phase preceding the early stage: the Project Initialization Phase. This phase includes all these elements that will enable a team to readily begin any project and to reducethe contingency of erroneously planned elements and of conflicts. The Project Initialization Phase aims to prepare a team in the best way for any project and wants to satisfy the old saying " Who good begins, ts in the middle ofthe work'. The guideline contains 35 activities, four of which can be split up in supplementary 49 tasks to be fulfilled by applying the two newly developed support methodologies: the Spiral model for marketing research and the integrated methodology for long term forecast. The guideline allows the project team to prepare a successful project or proposal for a radical innovation project. The Integration of Cooperation management,IP's management, basic marketing and project planning in the early stage of innovation management gives a comprehensive overview of all possible aspects of this particular typology of project in the medical device sector in Cooperation with lead users in academic environment. The guideline has been developed and tested in the context of a case project which had as an objectivethe development of a semiautomatictool for cardiac anastomosis (coronary bypass). The guideline and every Single methodology have been validated by observing and analysing 11 other radical innovation projects from the medical device sector. These projects were implemented by teams from university hospitals, from technical universities and in some cases by manufacturers. The guideline is valued as a very helpful and necessary tool for initialization of radical innovation projects in the academic environment, as it allows to make up for the missing business skills in academic projects without industrial partners. This guideline was designed to prepare the project network for a cultural coexistence, to avoid diversities between the teams to give rise to conflictual situations. As an example, the collaboration between engineers and surgeons, by reason of their completely different organizational and environmental cultures, was and still is a big challenge encountered in almost any project. L'embrione di un nuovo prodotto e sempre un'idea. Nel settore medicale, dopo la generazione di un'idea, inizia una fase di pre-studio. In molti casi la fase di pre-studio viene dedicata allo sviluppo di nuove tecnologie o all'adattamento di esistenti tecnologie, e queste attivita si focalizzano nella ricerca di specifiche funzionalitä del prodotto. Nel contesto dello sviluppo di prodotti medicali questa attivita viene chiamata "target research", che allunga significativamente la durata della fase di pre-studio, che tipico di questo settore si basa su progetti complessi svolti in coperazione tra diversi partner. L'idea del prodotto nata e inizialmenteapprofondita nell'industria o in una universitä (tecnica o di medicina), necessita di essere poi sviluppata con piu partner con lo scopo di poter contemporaneamente sviluppare o adattare tecnologie, sviluppare lo strumento stesso, e testarlo clinicamente (su animali o sull'uomo). Una semplice fase di pre-studio diviene perció un complesso progetto in una situazione di coperazione con contenuto "target research". Per questi motivi proponiamo di aggiungere una fase di initializzazione del progetto prima dell'inizio della fase di pre-studio. Il punto d'inizio della fase di inizializzazione del progetto e l'idea del prodotto generata dal team principale. La fase di inizializzazione del progetto e strutturata in quattro principali sub-processi: idea-refinement, concept and market exploration, partner identification and Cooperation planning. La fase di initializzazione vuole essere una guideline che ha lo scopo di supportare il team durante il primo stadio di ogni progetto contenente "target research" in innovazioni radicali nel campo medicale. Normalmente, chi ha avuto l'idea iniziale ed e il promotore di un progetto di innovazione radicale e il lead user, il quale frequentemente collabora giá negli stadi iniziale con un partner tecnico proveniente dal mondo accademico. Gli stadi iniziali di questa tipologia di progetto contenente "target research" si focalizzano principalmente sullo sviluppo di nuove tecnologie per specifiche funzionalita del prodotto, diventando cosi di media o lunga durata, e percio devono essere accuratamente pianificati in differenti aspetti come ad esempio la scelta dei partner per la coperazione, la gestione delle proprieta intellettuali, le prime importanti ricerche di mercato, la pianificazione del progetto, e i modelli di gestione dell'informazione e della comunicazione all'interno della rete di coperazione o esternamente. La guideline descrive la fase che procede gli stadi di pre-studio del progetto e prende il nome di "Project Initialization Phase", che include tutti quegl'elementi che permettono ai teams di iniziare nel meglio dei modi dei progetti riducendo significamente potenziali errori di pianificazioni e possibili future situazioni di conflitto nella rete di coperazione. La Project Initialization Phase ha lo scopo di preparare al meglio i differenti teams al progetto di coperazione come vuole il vecchio ditto "chi ben comincia, e a meta dell'opera". La guideline contiene 35 attivita, Quattro delle quail possono ancora essere descritte con 49 ulteriori tasks utilizzando le metodologiesviluppate quäle supporto alla guideline stessa: il "spiral model for marketing research" e il "integrated methodology for long term forecast". La guideline e stata sviluppata e testata nel contesto di progetto di riferimento il cui obbiettivo era lo sviluppo di un roboter semiautomatico per l'esecuzione di operazioni di bypass (anastomosi coronarie). La guideline e ogni singola metodologiae stata validata osservando e analizzando 11 progetti di innovazioni radicali nel settore medicale. Tutti questi progetti erano costituiti da reti di coperazione tra universitä tecniche e ospedali universitari, con rari casi di presenza giä negli stati iniziali di partner industriali. Questi progetti hanno valutato positivamente (molto d'aiuto e necessaria) la guideline specialmente in progetti in cui il partner industriale non era presente e mancavanole nozioni di bussiness e gestionali di base. La guideline e stata anche sviluppata con lo scopo di preparare e pianificare l'intero progetto in funzione delle differenti culture e strutture gerarchiche esistenti tra i diversi teams della coperazione. Una grande fonte di potenziali errori e conflitti in questa tipologia di coperazioni e dovuta infatti ai differenti punti di vista in differenti argomentiche possono avere ad esempio ingegneri e chirurghi.
